If you or a loved one in St. Louis County needs daily personal-care help, Missouri CDS may allow an eligible adult to choose and direct a qualified attendant. Harmony can discuss the program basics and how to take the next step.

Tell us where the person receiving care lives, whether Missouri has already assessed their needs, and whether they have chosen an attendant. A family member can contact us to learn about the process.
Choosing someone you know
Certain qualified relatives or friends may be eligible to serve as attendants. Missouri does not allow a spouse to be the paid CDS attendant. Required checks and program conditions still apply.
Confirm availability before making a change
If you already use another provider, ask about current Harmony participation and the state’s transfer process before changing your arrangements. An inquiry does not transfer services or guarantee a start date.
